Escalation note template: raise a problem so it gets solved fast
An escalation note template: the problem in one line, the impact, what you already tried, the options and the one decision you need, by when. Copy it and get a fast answer.
One line on the problem. The impact and the deadline. What you tried. Your options and your pick. The one decision you need, by when.
When to use it
When something will miss a date, cost money or hurt a customer, and you cannot fix it yourself. Escalating early is not complaining. It is giving someone time to help.

Tips
- Subject line says the decision and the date.
- Show what you tried. It proves you are not passing the problem up.
- Recommend an option. Managers decide faster with a recommendation.
- Escalate early. A week's notice gives options. A day's notice gives excuses.

Questions people ask
Will escalating make me look bad?
Not when it is early, calm and comes with options. Missing a date with no warning looks worse.
Should I copy the other person?
Usually yes, so nobody feels ambushed. Keep the tone factual.
Work you can trust, without redoing it.
Your team gets feedback before work reaches you, or your client.
