Meeting notes template: decisions, actions and owners
A free meeting notes template that puts decisions and action items first, each with an owner and a date. Copy it, see a filled example, or paste rough notes into the generator.
Decisions first. Actions next, each with a name and a date. Discussion last, and short.

Turn rough notes into this
Paste your messy notes, or a transcript, and get decisions and actions pulled out for you. It never invents owners or dates. If one is missing, it tells you.
Tips that make notes useful
- Write the action as a task. "Send the date to sales," not "sales comms."
- One owner per action. Two owners means no owner.
- Send the notes within an hour. Memory fades fast.
- Start the next meeting with last week's actions. That is how they get done.

Questions people ask
What is the difference between meeting notes and minutes?
Minutes are a formal record, often for boards. Notes are the working version for a team. This template works for both.
Who should take notes?
Not the person running the meeting. Rotate it.
Work you can trust, without redoing it.
